Read This If You’re on a Statin (or Considering One) 🫀🧬📉

This might change how you think about cholesterol, especially if you’ve been told that “lower LDL at all costs” is the whole heart-health story.
A new review and meta-analysis of nearly 200,000 patients across 20 randomized statin trials found that the relationship between LDL (or non-HDL cholesterol) reduction and actual cardiovascular outcomes is surprisingly weak.
Yes, statins reduce events overall, but how much LDL drops explains almost none of the reduction in heart attacks, strokes, or mortality.
In the review, lower LDL looked good on paper, but it failed as a reliable predictor of real-world outcomes. This reinforces earlier findings from a 2022 JAMA Internal Medicine analysis, which showed similar cracks in the LDL-centric model.
Now, nuance matters here. If you’re metabolically inflamed, insulin resistant, have existing heart disease, or have a strong family history of it, statins can absolutely be helpful.
But this study supports what I’ve said for years: LDL is a marker, not a master switch. Cardiovascular risk is driven by inflammation, endothelial function, oxidative stress, metabolic health, nervous system balance, sleep, and lifestyle—not just a single cholesterol number.
The big takeaway here is that lowering LDL alone isn’t the same as lowering heart disease risk.
